To place an order, please mail the NDDS Order Form, along with your cheque. (We are currently unable to accept C.O.D. orders. Payment using Visa or MasterCard is welcome.)

Request an order form to be faxed to you by contacting us by email or by telephone at (705) 752-5081 and ask for the Nipissing District Developmental Screen (Please remember to include your fax number in the message.)
